I came in the shelter as a OWNER SUR on 07/21/2016 from NY 10029, owner surrender reason stated was LLORDPRIVA.

07/21/16 12:16hrs Puchy is described as friendly, calm, and lovable. She has been receiving treatment for diabetes for the past year. She has lived primarily with adults however she has interacted with children ages 5 and 6 years old. Puchy is friendly with the children and will allow them to pet her. When Puchy has noticed other dogs outside she remains neutral. If they approach her she will sniff at them and will let the other dog sniff at her. BEHAVIOR Puchy has lived in a house and apartment the past year. She enjoys roaming free but will follow her owner around the home. She is constantly seeking attention from her family. She will roll onto her back for petting. Puchy isn’t bothered with having her food taken or touched while she is eating. She listens when instructed to get off of the furniture. She is accustomed to being bathed 1x a month. Puchy is allowed to roam free when she is left alone and is well behaved. Puchy will bark at sounds that she hears. When guest enter the home, she will greet them with a wagging tail. She behaves the same with strangers that she meets on the street. FOR A NEW FAMILY TO KNOW Puchy enjoys chewing on rawhide bones. Puchy will respond to the command “sit.” Puchy is house trained. She will remain in the home for up to 6 hours without having an accident. She enjoyed running loose in the backyard. She is walked at least 3x a day on leash. She was Purina dry dog food. She is given 6 units of her diabetes medication at 6:30 am and 6pm. She will sit still if given a treat while her medication is administered. Puchy will sleep near her owners’ bed at night. DURING INTAKE Puchy would remain lying down on the floor. She would come when called and would jump up on the legs of the person calling her. She allowed petting and being held. She was collared, scanned for a microchip(negative) and photographed.

Microchip:Scan negative Sex:Female Age:9 yrs old Mentation:Good Eyes:Curious/alert. OU cloudy. Ears:Errect,then goes in a down-position. Nose:Moist Teeth:Moderate-heavy dental staining. If abnormal BCS:N/A Skin:Okay Hair Coat:Okay Declawed:N/A Any injuries:Animal came into the facility as a owner surrender and as per memo has a mass. Animal is wearing a home-made shirt/slang covering area where there could be a posslble mass/mammary gland turmor in general. Did not remove for further investigation due to wanting to wait for vet to examine animal in a more thouroughly manner. Behavior:Very alert/hyper. Apppetite:Good Medication:Animal was stated in memo to be a diabetic as per owner and is currently on Insulin Novolin N 6units BID. Insulin was given this morning as per memo as well.
