I am a spayed female black terrier mix. The shelter thinks I am about 14 YEARS old.
I came in the shelter as a STRAY on 12/06/2016 from NY 10037, owner surrender reason stated was PET HEALTH.

12/07/2016 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 25.0 LBS.
Brought in for EHR but has microchip and previous owner. Geriatric. S/O) QARH. MM pk. EENT: Eyes – mod nuclear sclerosis, PLR +, no discharges. Ears cln but heavily matted. NND. Oral- missing most teeth, marked tartar, possibly painful L mouth but not really allowing visualization. MSI: BCS= 4/9, thin but hard to assess because hair is so matted. Matted clumps of hair with large patches of alopecia and some dermatitis (with erythema and lichenification) on back. CV: HR= 80, NSA (bradycardica at first but HR increases when dog moves), 3/6 murmur. L: clr bilat. Abd: SNP, NMP, appears thin. UG: FS, no MGT’s. Eating – food bowl empty. A) Severe dental dz. Hair matts with dermatitis. Heart murmur. P) Clavamox 125 mg PO BID for dermatitis and ddz. Dog needs shaving but would not recom sedation. Consider rescue or EHR. Overall condition – fair to poor. PROGNOSIS: Guarded for heart disease pending diagnostics. Guarded to poor for ddz. Sedation for dental tx would be a risk. Consider BW.
