Medical Summary Estimated age: 11yrs Microchip noted on Intake? no O/S due to owner moving to residence that does not allow pets that bark. Dog is BARH, immediately snapped at my hand when I went to perform exam, muzzled, required little to average restraint for rest of exam, seemed to growl/groan when abdomen palpated (guarded) T = NA P = wnl R = wnl BCS 5/9 EENT: Eyes appear cloudy OU (r/o NS vs cataracts), ears clean, no nasal discharge noted Oral Exam: not performed PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: guarded, unable to palpate adequately U/G: wnl MSI: Ambulatory x 4, skin free of parasites, approx 1cm scab right lumbar area, fell off revealing superficial excoriation underneath CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Geriatric healthy Plan: Acceptable candidate for adoption or rescue pending behavior eval. Prognosis: Good-Fair SURGERY: Already neutered
