Medical Summary DVM Intake Exam Estimated age: 3yrs Microchip noted on Intake? no History : O/S Subjective: Dog is BARH. Observed Behavior – Dog muzzled during intake. Stiff posture, wagging tail. Higher-level warnings follow lower-level warnings (stiffens, then head flips with certain type of approach). Dog repeatedly jumps up on and grabs at handler using front paws. Started deep growling and fighting restraint when trying to hold off vein for blood draw; will schedule HW test for time of neuter. Objective T = NA P = wnl R = wnl BCS 4/9 EENT: appear grossly normal from a distance Oral Exam: NE (muzzled) PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: NE U/G: wnl; intact male, both testicles palpable MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Apparently healthy Plan: Acceptable candidate for adoption or rescue pending behavior evaluation. Okay to schedule neuter. Prognosis: Good-Excellent SURGERY: Okay for surgery
