DVM Intake Exam: Estimated age: 8-10years Microchip noted on Intake? no History : stray Subjective:severely emaciated Observed Behavior -calm, allows handling Evidence of Cruelty seen -possible Evidence of Trauma seen -no Objective T =100.6 P =80 R =wnl BCS 1/9 EENT: ou sclerosis, ears clean, no nasal or crusty ocular dc Oral Exam: pale, cold gums PLN: No enlargements noted H/L: NSR, NMA, , Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: no SS seen MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities bloodwork;mild anemia, blood sugar >650 Assessment: diabetes Prognosis:guarded to fair Plan: IV catheter maintenance fluids 30ml/hour small meals only cbc/chem waive rabies vax until stronger. gave 2 units humulin N at 7pm SURGERY: Permanent waiver due to age

11/08/17 Vet Notes : Hx: severe emaciation, severe hyperglycemia on intake; given 3U Humulin insulin SQ last night at 7 pm and started on LRS at 30 ml/hr
S/O
-great appetite, pu/pd
-appears depressed in cage initially, after 200 ml fluid bolus over 30 minutes, she was much more interactive and walking around, very sweet and docile; does not react to other dogs in the room
Glucometer reads High this morning, indicating BG >700
-mm pk, tacky; CRT ~2 sec
-OU: mild mucoid discharge, immature cataracts
-eupnic, quiet lung sounds; possible grade II L parasternal murmur
-abdomen slightly tense but no palpable abnormalities
-severe emaciation

Diabetes mellitus, uncontrolled
Mucoid ocular d/c-r/o KCS vs allergic conjunctivitis vs other
Severe emaciation-suspect due to uncontrolled diabetes

Switch to NaCl, continue IVF at 50 ml/hr
cerenia 1.5 ml IV SID
humulin 4U SQ BID with food
prognosis: guarded to fair; does not appear to be ketoacidotic and is eating which improves prognosis but is severely emaciated and has a long road to recovery with frequent BG checks; will require insulin therapy indefinitely

-QAR in cage but much brighter overall, barking in cage for attention; wagging tail when out of cage
-good appetite, eating i/d well
-normal bowel movement, large urination this morning
-mm pk, tacky; CRT 2 sec
-no nasal discharge or sneezing
-OU: open and clear of discharge today; immature cataracts
-eupnic, no murmur noted
-soft, doughy abdomen
-severely emaciated, likely due to uncontrolled diabetes

Continue humulin insulin (4U SQ BID), famotidine and cerenia IV
TID feeding with i/d-8 am, 2 pm, 8 pm
recheck BG 6 hours post insulin today
recommend BG curve in 10-14 days
prognosis: fair; appears to be making significant improvements but has a long road to recovery and will require frequent blood work monitoring and indefinite insulin therapy

Look: 1. Dog’s eyes are averted, ears are back, tail is down, relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands.
Sensitivity: 1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.
Tag: 1. Dog stands with tail low and wagging, and comes toward the Assessor in a friendly manner when the Assessor ceases moving.
Paw squeeze 1: 1. Dog does not respond at all for three seconds. Eyes are averted and ears are relaxed or back.
Paw squeeze 2: 1. Dog gently pulls back his/her paw.
