Medical Summary DVM Intake Exam Estimated age: 10 Microchip noted on Intake? no History : stray Subjective: BAR, hydrated Observed Behavior – quiet, allows exam/handling Objective P = 120 R = 20 BCS: 7/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mild staining and wear, pale/pink mm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female intact, no mgts or dc MSI: Ambulatory x 4, severe flea infestation and flea allergy dermatitis, no masses noted, dirty matted coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: geriatric with severe flea infestation and flea dermatitis Plan: Flea bath as soon as possible grooming/shave down consider screening bloodwork to rule out anemia secondary to flea infestation continue to monitor at MACC Prognosis: good with treatment and supportive care SURGERY: permanent waiver for surgery due to age
