Medical Summary Scan negative Hx: Owner surrender. Many health conditions developing in last few years. S: Quiet, blind, allows all handling without resistance O: QAR-H, MMs pink and moist, CRT <2 sec EENT: Moderate tear staining, with serous discharge and crusty debris at medial canthus OU. OD: Complete cornea opacity, cornea or anterior chamber appears red-tinged. Conjunctival injection. Mild exophthalmos. Fluorescein stain: negative. IOP with TonoPen: 29. OS: cataract, either almost fully mature (85%) or hypermature and reabsorbing. Conjunctival vessels injected. Fluorescein stain: negative. IOP with TonoPen: 13 (difficult to read with first few attempts). No discharge AU, nose. Severe tartar and gingival recession. PLNs: Not enlarged. H/L: Resp sinus arrhythmia, NMA. Eupnic, quiet lung sounds. Abd: Tense, no pain on palpation, no masses palpated M/S/I: Amb x4. There are two large bulging masses on the perineum: one mass is at the 6:00 position relative to the anus, firm, about 3″ dia, and firmly adhered to the underlying tissue. The second mass is caudal to the first, about 3″ dia, softer than the first, and slightly pedunculated. The vulva hangs just below the second mass. UG: Female Rectal exam: The dorsal mass is palpable in the rectal canal, but no other abnormalities palpated. A: 1. Severe ophthalmologic disease – OD corneal disease and glaucoma (possibly secondary to phacolytic lens-induced uveitis although we cannot assess the lens), OS cataract, possibly hypermature and possibly also causing LIU. 2. Severe dental disease 3. Large perineal masses – R/O perineal adenoma vs. adenocarcinoma vs. other Short-term prognosis: Fair to poor (with treatment) to guarded (without treatment) P: 1. Recommend transferring to specialty clinic where complex eye conditions can be managed appropriately. 2. Rec FNA of perineal masses 3. Consider dental cleaning, rads and extractions 1088.
