History : owner surrender because owner does not have time to care for him Subjective: BAR Observed Behavior – a little anxious but docile and allows handling  BCS 5/9 EENT: OU-mild nuclear sclerosis, no nasal discharge, AU-clear Oral Exam: mm pk/pigmented, moist; severe dental disease PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G:male intact, 2 scrotal testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, hair coat overgrown with some matting CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Dental disease Plan Rec’d dental cleaning with likely extractions after placement Prognosis: good SURGERY: Okay for surgery
