Medical Summary DVM Intake Exam Estimated age: Estimated 7-10 years based on dentition, alopecia and overall condition Microchip noted on Intake? Scanned neg on intake History : Stray, was being cared for on the roof of a building Subjective: Alert, walks well on leash Observed Behavior – Allows full exam. Becomes very tense for vaccines and blood draw. Does NOT like muzzle, became very stressed out. Restrained successfully with large towel around neck. Objective BAR, BCS 3/9, MMs pink and moist EENT: Alopecia on dorsal skull. Eyes clear, ears clean, no nasal discharge noted Oral Exam: Moderate tartar PLN: No enlargements noted in popliteal or mandibular LNs H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, developed vulva – intact. Irregular firm mammary masses about 2 cm dia – one associated with the 4th left mammary gland, one associated with the 4th right mammary gland, and one midway between the left and right 2nd mammary glands. MSI: Alopecia/thin fur of ventrum, legs, and head. Skin in these regions is quiet. Medial patellar luxation of left stifle – grade 2/4. Ambulatory x 4, skin free of parasites, no other masses noted. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment: 1. Mammary masses 2. Emaciated 3. Alopecia – R/O metabolic cause, alopecia X, thyroid disease, autoimmune, other 4. Mild MPL 5. Dental disease Plan: CBC/chemistry – all WNL Recommend FNA of masses prior to removal Prognosis: Fair SURGERY: Temp waiver for alopecia and emaciation 1088
