Estimated age: 8 years; Microchip noted on Intake? no; History : surrender; Subjective: BAR, hydrated; Observed Behavior -very friendly, allows exam and treatment with minimal restraint. ; Objective; P = 120 ; R = wnl ; BCS= 5/9; EENT: Eyes clear, ears clean, no nasal discharge noted; Oral Exam: grade 2-3 periodontal disease; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: female intact, no mgts or dc; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. Mild skin fold pyoderma noted above snout.; CNS: mentation appropriate – no signs of; neurologic abnormalities; Assessment: in apparent good health; Prognosis: good; Plan: cleaned skin fold pyoderma with chlorhexadine solution, applied triple antibiotic ointment afterwards. continue to monitor at ACC; SURGERY: permanent OHE waiver due to age
