I am a female white and brown shih tzu mix. The shelter thinks I am about 9 YEARS old.
I came in the shelter as a OWNER SUR on 07/24/2017 from NY 11433, owner surrender reason stated was OWNER SICK.

07/26/2017 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 16.6 LBS.
Hx: Mammary mass – has been present for last 6 months but was small at first – started to grow about a month ago. S: Calm, relaxed, allows all handling O: BAR-H, BCS 5/9, MMs pale pink and moist EENT: No discharge OU, AU, nose. Moderate tartar H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. Large firm mammary mass associated with cranial right mammary gland – 4-5 inches dia – irregular shape, many pustules, cavitations with moist purulent discharge. UG: Female intact Neuro: Alert and appropriate, no sign neurological deficiencies A: 1. Mammary mass 2. Dental disease Short-term prognosis: Fair-guarded. Suspect malignancy due to necrosis and infection, as well as rapid growth P: 1. Start Clavamox 125 mg PO BID x10 days 2. Simbadol 1 ml PO SID x10 days 3. Recommend FNA of mass, chest rads – consider mastectomy 1088

07/24/17 17:37 Basic information Veevee is a female white and brown small breed mix with a mass and possible skin conditions. Owner has had Veeve since she was 1 years old and is surrendering because of owner health. Socialization Veevee Has lived with children in the past but not recently. When around children now Veevee will ignore them. Veevee plays gently with adults. Veevee has lived with another dog and was playful with him. Veevee played gently with the other dog. Around dogs outside Veevee will bark and sniff them. Veevee has not lived with cats. Veevee does not have a bite or scratch history. Behavior during storms and fireworks Veevee is nervous and will pace around the house. Veevee isnt bothered when having her food bowl touched while eating and having toys taken away. When trying to take away a bone Veevee will lock on it and not let go but will not growl or try to bite. Veevee does not go on the furniture at home. Veevee isnt bothered when being held or restrained. Veevee is use to getting bathes and tolerates them. Veevee enjoys getting brushed and will stretch out for you. Previous owners never attempted to trim Veevees nails. When someone unfamiliar approaches the house Veevee will bark. For the new family to know Veevee was described as friendly affectionate and playful with a high activity level. Veevee enjoys playing with balls and knows how to play fetch. When home with Veevee she will follow you around. Veevee is mostly house trained and wee wee pad trained. Veevee will only have accidents in the house when someone doesn’t walk her before leaving the hosue. Veevee Veevee goes on 2 walks a day and prefers to go to the bathroom on grass. Veevee doesn’t pull on the leash and has never been let off the leash in NYC. Veevee knows the commands sit and give paw. Veevee eats soft dry food of any brands and usually has the bowel left out for her. When its time for bed Veevee sleeps anywhere in the house or on the bed. Behavior during intake. Veevee was a little shy during intake. She was afraid of the scanner but allowed the counselor to scan and collar her. Veevee allowed all handling.

Other notes: VeeVee may pace during storms. SAFER ASSESSMENT: Date of assessment 7/26/2017 Look: 2. Dog’s eyes are averted. Her body trembling; her tail is low and not moving. She allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture, mouth closed Tag: 1. Follows slowly at end of leash, body soft Squeeze 1/ 2: 1. Dog does not respond at all for three seconds. Her eyes are averted and her ears are relaxed Toy 1. Minimal interest.
