SAFE 01/18/18

VAN HALEN – 11583

5 yr. male black/brown, stray, 10 pounds:

DVM Intake Exam; Estimated age: ~5-6 years; Microchip noted on Intake? negative; History : stray ; Subjective: BARH; Observed Behavior – Very painful, crying, and yelping before sedation. Difficult to assess behavior due to pain; Evidence of Trauma seen – yes; Objective ; P = wnl ; R = eupneic ; BCS 5/9; EENT: Eyes clear, ears are erythematous with mild waxy debris, no nasal discharge noted; Oral Exam: adult dentition with mild to moderate dental tartar, no oral lesions noted; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: MI, 2 testicles descended; MSI: Ambulatory x 3 with RHL lameness, skin free of parasites, no masses noted, healthy hair coat; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment: Complete closed short oblique fracture of the right distal tibial and fibular metaphyses with craniolateral displacement; Mild dental disease; Plan: Hydromorphone 0.1mg/kg IM BID (ideally every 4-6 hours) x5d until 11/8-one dose given on intake; Dexdomitor 10mcg/kg IV sedation for rads; 2 view RHL rads-Complete closed short oblique fracture of the right distal tibial and fibula metaphyses with craniolateral displacement; Rimadyl 4.4mg/kg SQ SID x5d until 11/8; Placed lateral splint-stirrups, cotton padding, splint, cotton padding, vetcling, vet wrap, elasticon; -Unable to manually reduce fracture; Recheck daily-splint change every 2 weeks (sooner if soiled) but recommend surgical repair; Prognosis: Good with surgical repair; SURGERY: temporary waiver from surgery due to injury

PROFILE:

Medical Notes: Van Halen arrived at ACC with a broken back leg.

Van Halen looks to be a 5 year old small black and brown dog that was brought to the ACC as a stray.  When meeting that Staff at the ACC he came up to the right away and allowed them to pick him up.

Behavior Notes:During intake Van Halen allowed the counselor to pick him up, collar him and take his picture with no problems.
