Medical Summary 06/11/17 11:33 DVM Intake Exam Estimated age: 3-5 yo Microchip noted on Intake? no History : stray Subjective: Observed Behavior – nervous, resists being removed from kennel on lead, but follows treats out and eats gentlyfrom hand, allows full exam with no muzzle, muzzled for proptosed eye exam Trauma seen – yes Objective T = P =90 R = 10 BCS 5/9 EENT: AU clear, no nasal discharge, OD WNL, OS proptosed globe rotated laterad with dry non functional eye, able to minimally retropulse globe, orbit around OS globe swollen Oral Exam: minor dental tartar, no pain on opening the mouth or palpation of jaw PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: tense, difficult to palpate U/G: swollen vulva with no discharge, lactating MSI: Ambulatory x 4, dirty haircoat, mild swelling at L muzzle, no bruising CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Assessment lactating r/o postpartum vs late pregnancy vs other proptosis Plan 1.1 ml Hydromorphone SQ BID x7d cbc/chem thoracic rads NH place vs in house enucleation and spay Prognosis: SURGERY: Temporary waiver due to requires enucleation of Left eye
