Medical Summary DVM Intake Exam Estimated age: ~ 10-12 mon Microchip noted on Intake? scan negative History : owner surrender Subjective: BARH Observed Behavior – nervous, tail tucked, mild trembling, allowed PE but resists restraint and a lot of handling , but limping reported Objective P = wnl R = wnl BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: nsf PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female MSI: Ambulatory x 4, tense behav. but no limping observed, allowed palpation and flex/extend of limbs, small healing abrasion to the lateral side of right tarsus; skin free of parasites, no masses noted; small spot of alopecia on tail, mildly dirty coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: mild dermatitis, otherwise healthy Plan: rec’ medicated baths – montiro and recheck for limping Prognosis: good SURGERY: Okay for surgery
