Medical Summary DVM Intake Exam Estimated age: 4 years Microchip noted on Intake? neg History : owner surrender due to recent destructive behavior Subjective: BAR Observed Behavior – initially appeared friendly and energetic; allowed exam but growled and lunged during vaccination  Trauma seen – abrasions on face Objective BCS 5/9 EENT: OU-mod episcleral injection AS-mod erythema, dark waxy discharge AD-moderate to severe lichenification with yellow crusting discharge Oral Exam: mm pk, moist; CRT <2 sec; mild to moderate tartar/staining PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact; 2 scrotal testicles MSI: ambulatory x 4 with no lameness noted abrasions/poss superficial bite wound along R side of face-crusting, no signs of infection, appears several days old; mod erythema and old, dried blood staining on fur along neck CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Suspect bite wounds on face otitis externa episcleral injection-r/o behavioral vs allergic Plan cephalexin 1000 mg PO BID x 10 days Osurnia AU, 2 doses Prognosis: excellent SURGERY: Okay for surgery
