Medical Summary Microchip: negative Sex: intact male Age: appx 5y Mentation: BARH Eyes: clear Ears: clean Nose: no d/c Teeth: heavy staining w/ tartar although appears to be poor diet as exposed areas are white If abnormal BCS: underweight Skin: WNL Hair Coat: heavily matted throughout and urine stained coat, shaved off mats and bathed to warm dog, needs touch up groom and picture when dry Declawed: N/A, overgrown (trimmed) Any injuries: no; when initially brought to medical dog was shivering uncontrollably and ataxic, soaked in water, temp was 97F and external heat was used; dog has knuckling and occ. cross stepping in hinds (unclear if muscle wasted/atrophy or neuro), no pain on palp, slight crepitus in stifles Behavior: very sweet and friendly, wagging throughout, attempts to come to handler when called Medication: N/A, preventatives given
