Medical Summary Sex: Female Scan: Negative Estimated age: 12 years old based on dentition, weight and cataracts Fleas: No flea dirt. S: Quiet, stands in one place and allows all exam. Whines when vaccinated. O: Quiet, passive, BCS 1/9, MMs pink and moist, CRT <2 sec EENT: Nuclear sclerosis or cataracts OU (did not perform ophthalmoscopic exam). Pt acts as if blind. Mild mucoid discharge OU. Clean AU, nose. Severe tartar and gingival recession, especially on L maxillary canine tooth. Many teeth missing. Mats all over face and muzzle. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Tucked up, no pain on palpation. M/S/I: Amb x4. Pt stands with hind legs slightly flexed at the hips, knees and hocks, but seems to have good range of motion in all of these joints. The front legs are also flexed at the elbow and shoulder, and there is poor extension of the shoulders. Lumbar kyphosis. Pt has difficulty lying down and then standing up once lying down. Fur coat very matted and unkempt. Anus is erythematous. UG: Female, developed vulva. Neuro: Stands facing a corner of the cage and doesn’t move. Occasionally lifts head high in the air so nose is directly up. Thin brown discharge on blanket in cage – cannot tell if it is coming from fur on face, or from anus. A: 1. Severe dental disease 2. Emaciated 3. Heavily matted 4. Likely cataracts and blindness 5. Stands with flexed legs, poor shoulder extension, kyphosis – R/O arthritis vs. old fractures vs. other 6. Facing the cage wall – R/O CCD vs. brain tumor vs. blind/arthritic vs. other Short-term prognosis: Poor-guarded P: 1. Administered Activyl 4-14# and pyrantel 0.7 ml PO during exam 2. Needs grooming 3. Third feeding sign 4. LRS 125 ml SQ SID x5 days 5. Buprenorphine 0.17 ml SQ BID x5 days, reassess to determine if pain medication helps pt’s mobility, and if buprenorphine or an NSAID should be given long-term 1088
