DVM Intake Exam; Estimated age: approx. 8yo; Microchip noted on Intake? neg ; History : stray; Subjective: BAR, friendly, good for medical handling. Observed Behavior – Objective ; T =; P = wnl; R = eup; BCS = 4/9; EENT: mild mucoid discharge OU, brown waxy debris AU, no nasal discharge noted; Oral Exam: mod to severe dental disease; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: intact female, no spay scar seen, no MGTs, no vulvar discharge; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat with patchy areas of hypothrichosis. Old scab wound on right hip that shaved off easily, wound underneath was fully healed. CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: normal externally; Assessment; geriatric FI medium mixed breed canine; hypotrichosis — r/o old age vs endocrine vs other; Prognosis: fair, pending results of bloodwork; Plan: CBC/superchem; ctm at MACC until transfer/adoption; SURGERY: Permanent waiver due to old age
