Medical Behavior Evaluation BLUE
Medical Summary Microchip: Negative Sex: Intact male Age~ Approx- 8 yrs Right eye cataract Wound ventral to the left eye cryptorchid dog BAR AMBX4 Ears: Clean/ wnl Nose: Clear / wnl Teeth: moderate dental tartar BCS: 3 Skin: APH / WNL Hair Coat: WNL/long Behavior: tense, nervous and not allow to handle during exam NOSF
Weight 15.4

Re Exam:

09/12/16 : s/o: large laceration under left eye–skin is hanging off preorbital area, OD has opaque white cornea and excessive discharge; dog appear blind–bump into walls , unaware of surrounding; uniolateral cryptorchid, SQ; SQ testicle very large; clean teeth–may be younger than 8; heart murmur 4/6

a: hm, blind, eye wound, cryptorchid

p: recc immediate transfer to facility that can repair periorbital wound and evaluate heart

dog with also need complete ophtho exam and eventual removal of retained testicle with biopsy; fair/guarded prognosis; gave 0.4 buprenorphine (.5mg/ml); and 0.2 meloxicam (5mg/ml); amoxiclav bid 1.8ml (.45mg/ml)

9/12 : Tommy did not eat during the night shift.
