Medical Summary Estimated age: 1-2yrs Microchip noted on Intake? no. O/S due to unable to care for dog; reported to have forelimbs missing since birth. Dog is Q/BARH , shy/timid, little restraint required for exam, allows all body part to be touched T = NA P = wnl R = wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Grade 0-1/4 dental dz; pronounced underbite PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: reported to be neutered; no palpable testicles MSI: amb x 2 (missing both forelimbs just proximal to elbows), skin free of parasites, no masses noted, reddish-brown staining of paws and forelimb stubs; dry flaky skin CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: congenital defect(s)-missing forelimbs, mild dermatitis (ruleout poor husbandry vs food allergy, atopy, etc.) Plan: Recommend rescue placement or with adopter willing to take on “special needs” dog. Dog should be fit for wheelchair. Prognosis: Good with appropriate care SURGERY: Already neutered
