Medical Behavior Evaluation GREEN
Medical Summary QAR; scan negative; male intact~ 4 months; emaciated, dehydrated, weak and malnourished; gave soft food and water; great appetite, drank lots of water; swollen left mandibular lymphnode; old wound on forehead; some soptted alopecia or old wound on head, ears; occular d/c, eyes cleaned; very dirty coat; gave 200cc SQF; placed on heat pad and comford beeding in medical; NOSF
RE-EXAM:
Hx: Found in an apartment building, apparently running around on the stairs. S: Quiet, scared, shrinks to ground and does not move. Allows all handling. O: QAR, BCS 1-2/9, Ms pale pink and tacky, mildly decreased skin turgor, estimated 5-8% dehydrated; EENT: Mild mucoid and crusty discharge OU. Clean AU and nose. Mild food debris on teeth – adult teeth in the process of erupting. Large superficial injury between eyes (1-1.5 cm dia), scab peeling off and below it, granulation tissue and mild discharge is visible. Spotty alopecia all over face and especially in periocular region OD. Ear margins are heavily crusted and peeling, with ulcerated tissue underneath. Large firm swelling in the region of the left TMJ. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, large bladder, no masses palpated. M/S/I: Unwilling to walk but all legs appear to be functional. Smells like diarrhea; diarrhea crusted on hind legs, pelvis and tail. Pressure ulcer on left iliac crest with diarrhea inside it. UG: Male intact, testicles soft and symmetrical. WT : 15lbS

- Emaciated
- Facial dermatitis – R/O ringworm vs. trauma vs. fleas vs. other
- Unwilling to walk – R/O pain vs. fear
- Wound on face, pressure ulcer on hind legs
- Ocular discharge – R/O CIRDC vs. other
Short-term prognosis: Fair to poor; Current surgery status: Temporary waiver for wounds and emaciation.
P:
- Feed (third feeding) and offer water free choice
- Clavamox 125 mg PO BID x10 days
- Consider DTM culture
- Bathe and lyme dip q3 days
