Medical Summary DVM Intake Exam Estimated age: 2 years Microchip noted on Intake? no History : stray Subjective: bar, hydrated Observed Behavior – a little timid, painful from injury to right rear leg, but easily examined with minimal restraint. Objective P = 120 R = 20 BCS: 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: negative oral PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female intact, no mgts or dc MSI: Ambulatory x 3, toe touching lame right rear. No open wounds, skin free of parasites, no masses noted, healthy hair coat. Suspect right femoral fracture. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: 2 year old, female intact terrier mix, non-wgt bearing lame right rear, rule out fracture vs luxation vs other. Appears to be stable and in good health otherwise. Plan: Given 0.7 ml of buprenorphine i.m., given 0.5 ml carprofen SQ Good sedation and analgesia for radiographs. Xrays: proximal short oblique fracture of right femur. Possible mild Sacro-iliac luxation on right. No other fractures noted. Chest: mild sternal lift of cardiac shadow (mild pneumothorax?) Recommend surgical/orthopedic evaluation. continue analgesia as required (hydromorphone/NSAID) continue to monitor at ACC Prognosis: good with internal stabilization (plates or pins) SURGERY: Okay for surgery
