DVM Intake Exam; Estimated age:10y; Microchip noted on Intake?n; History :seized; Subjective:bilateral len luxations. visions probaly affected; Observed Behavior -nervous. will threaten to bite when head approached but not follow through; Objective ; T =; P =80; R =wnl; BCS 5/9; EENT: ou lens luxation, ears clean, no nasal or ocular discharge noted; Oral Exam:difficult to examine. oligodontia and dental disease present; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G:2 testes; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: lens luxation, dental disease; Prognosis:good; Plan:no treatment at this time, dental in future; SURGERY: Permanent waiver due to age
