Estimated age: 10; Microchip noted on Intake? n; History : OS; Subjective: BARH; Observed Behavior – very timid/scared; Objective ; T = -; P = 100; R = wnl; BCS = 4.5/9; EENT: Immature cataracts, ears clean, no nasal or ocular discharge noted; Oral Exam: Edentulous ; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: No spay scar – suspect intact; MSI: Grade 1 left medial luxating patella, Ambulatory x 4, skin free of parasites, no masses noted, flaky hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: NP; Assessment: Geriatric, immature cataracts, edentulous ; Grade 1 left medial luxating patella not affecting gait, otherwise suspect healthy; Anxious in ACC so will place on anxiolytics for 3 days ; Prognosis: Excellent ; Plan: Give intake txs; Place on trazadone 50mg SID PO for 3 days
