Medical Summary Sex: Female, small vulva so likely spayed Scan: Negative Estimated age: 14 years per former owner Fleas: Moderate amount of flea dirt. S: Quiet, docile, allows most handling and nail trimming but jumps away from vx and oral exam O: QAR-H, BCS 6/9, MMs pink and moist, CRT <2 sec EENT: Nuclear sclerosis or cataracts OU (ophthalmoscopic exam not performed). Mild serous discharge from nose. Mild wax in ears AU. Severe tartar, gingivitis and gingival recession. One black crusty mass (1-2 cm dia) and one cyst (0.5-1 cm dia) dorsal to OD. H/L: NSR, NMA. Quiet lung sounds. Abd: Slightly tense, no pain on palpation M/S/I: Severe medial buttressing, muscle wasting in hind end, stiffness in hind legs when walking, medial patellar luxation grade 4/4 bilaterally. Very long nails on all feet. Large skin folds in inguinal region. Flea dirt. UG: Female, small vulva and nipples so likely spayed. A: 1. Severe dental disease 2. Arthritis and muscle wasting in hind legs secondary to patellar luxation, difficulty walking 3. Long nails 4. Multiple small masses on head 5. Serous nasal discharge – likely secondary to dental disease and nasal irritation or possible oronasal fistula. 6. Flea dirt. Short-term prognosis: Fair with appropriate pain mgt P: 1. Gave 1.5 ml pyrantel and Pet Armor for dogs up to 22 lbs during exam 2. Trimmed nails during exam 3. Long-term medical care must include dental cleaning, radiographs and likely tooth extractions, as well as pain management for both stifles. 1088
