DVM Intake Exam; Estimated age:12-14 years; Microchip noted on Intake? positive; History :geriatric dog found wandering around park ;Subjective:QAR; Observed Behavior -docile, allows all handling; Objective ; BCS 5/9; EENT: -OU: mature/hypermature cataracts, very limited vision; -no nasal discharge; -AU: clean; Oral Exam:mm pk, tacky; CRT <2 sec; severe dental disease ; PLN: No enlargements noted; H/L: grade V-IV/IV heart murmur; mild increase in inspiratory effort; mild pulmonary crackles dorsally; ABD: Non painful, no masses palpated; U/G: neutered male; MSI: -ambulatory x 4; -pain on palpation of TL spine; -sebaceous gland cyst of top of head with mild irritation; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: grossly normal; Assessment: 1. Heart murmur, CHF; 2. Cataracts; 3. Back pain-r/o IVDD vs other; 4. Dental disease; 5. Sebaceous cyst; Prognosis:fair; Plan: Gave 0.2 ml torb IV for radiographs; Gabapentin 50 mg/ml: 0.8 ml PO BID ; CBC/chem; Chest radiographs; -gave 0.4 ml lasix 50 mg/ml SQ after radiographs; will continue 0.2 ml SQ BID; -recommend cardiology consultation after placement, will likely require additional medications including ACE inhibitor and pimobendan (not currently in stock in shelter); SURGERY:neutered
