DVM Intake Exam; Estimated age:10 (approx) Microchip noted on Intake?N Microchip Number (If Applicable): History : stray; Subjective:bar; Observed Behavior – blind ou, cautious, may be more resistant to restraint due to blindness; Objective ; T =; P = wnl; R = reverse sneeze/stridorous upper airway; BCS 4/9; EENT: mature cataract od, complete pigmentary keratitis os w/ sl exophthalmus and vascularization, dry, brownish ear d/c, au, no nasal or ocular discharge noted, but obstructive upper airway noise; Oral Exam: mild staining, overall teeth in good condition; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: mi x 2; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal:nw; Assessment: approx 10yr small mixed breed mi; blind – mature cataract od, pigmentary keratitis – r/o glaucoma; upper airway – r/o reverse sneeze vs mass vs elongated soft palate vs other obstructing structure; otitis externa au; Prognosis: good – pending further exam of upper airway; Plan: cbc/chem prior to sedation tomorrow to examine upper airway for obstruction (mass/polyp) vs elongated soft palate; tresaderm au bid 5 drops; tab+H ou bid (stained negative) wanted to do tonometry but covers for tonopen could not be found in-house on Sunday. SURGERY: Temporary waiver due to age
