SAFE 08/06/16

Manhattan Center

My name is TAPU KOKO. My Animal ID # is A1083159.
I am a female white and tan pekingese mix. The shelter thinks I am about 7 YEARS old.

I came in the shelter as a STRAY on 07/28/2016 from NY 10473, owner surrender reason stated was STRAY.

MOST RECENT MEDICAL INFORMATION AND WEIGHT
07/31/2016 Exam Type RE-EXAM – Medical Rating is 3 NC – MAJOR CONDITIONS NOT CONTAGIOUS, Behavior Rating is NH ONLY, Weight 22.8 LBS.

VC – lameness, corneal scar OS: pigmentation at medial portion of cornea, central opacity (4 mm diam) with mild depression/defect. No active discharge. OD: neovascularization present Fundic WNL OU; no evidence of active inflammation or infection OU. Eyes slightly bupthalmic (breed) Unable to perform orthopedic exam d/t temperament – became fractious and attempted to bite multiple times. Hind limb weakness observed inconsistently. Occasionally slips hind limbs, mild knuckling A: Suspect previous corneal injury vs other ongoing chronic corneal disease Hind limb weakness – open P: Reco placement for full work up of issues, likely with sedation Consider Rimadyl if lameness persists – appears more as weakness related than pain currently Prognosis open Tapu Koko was found as a stray so behaviors in the home are unknown. During Tapu Koko’s initial medical exam, she was nervous and will snap if touched behind the head or anywhere past hips. Due to her hind limb weakness and dragging of hind legs, all tactile portions of the SAFER assessment were skipped. Tapu Koko showed minimal interest in Toy, sniffing a few times. When off leash with other dogs Tapu Koko slowly wanders the room, mostly just standing around. Tapu Koko must be placed with a New Hope partner. Toy 1. Minimal interest. Dog smells, licks and mouths, then walks away. Dog-dog: When off leash with other dogs Tapu Koko slowly wanders the room, mostly just standing around.
