Medical Behavior Evaluation BLUE
Medical Summary DVM Intake Exam Estimated age: 7-10 years based on dentition, heart murmur and overall condition Microchip noted on Intake? Yes History : No hx given, came in with 1 other dog Subjective: Alert, BAR Observed Behavior: Anxious and whines when not being paid enough attention. Shrieks during vx. Allows full exam. Objective BCS 5/9, MMs pink and moist EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Many teeth missing, tartar on remaining teeth. Tongue hangs out to right side due to missing teeth. PLN: No enlargements noted H/L: Grade 5/6 systolic heart murmur, PMI left apex, thrill present. NSR, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, spay scar visible, undeveloped nipples and vulva. MSI: Medial patellar luxation – grade 2/4 in both stifles. Ambulatory x 4. Skin free of parasites, no masses noted. Some small comedomes on dorsal feet. CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: 1. Severe heart murmur 2. Dental disease, missing teeth 3. Medial patellar luxation Plan: 1. Rec cardiologist evaluation – echo or chest rads 2. Monitor stifles – may not need any treatment at all, or may need chronic pain medication Prognosis: Fair-good 1088
