09/19/16 12:38 Basic Info: Symba A1090418 is a 10yr old male tan Poodle mix who was given to the owner as a gift from a friend 10yrs ago and is being surrendered due to pet health and the cost of medication(s). Symba was last seen by a veterinarian in early 2016 and currently has a history of seizures. Socialization: Around strangers Symba is friendly/outgoing. He’s spent time with young children-teenagers and was gentle, exuberant & playful with them and the the adults in the home. Symba hasn’t lived with other animals but has spent time with some during walks or at the park and would try to pull the owner towards them or ignore them. He hasn’t bitten/scratched any person or animal. Behavior: Symba has accidents around the home at times; the owner thinks it’s due to his being older. He isn’t bothered by the following: having his food/food bowl, toys/treats touched/moved, being moved off of furniture, being held/restrained or if someone unfamiliar approaches the home/family member. Symba hasn’t been disturbed while sleeping/resting or had his nails trimmed. For A New Family To Know: Symba is described as friendly, affectionate, playful & mellow with a medium activity level. He enjoys playing & resting. When the owner is home Symba follows them around. He enjoys ball, squeaky & stuffed toys (not so much anymore). Symba is a mostly indoor only dog who’s walked 2-3x a day. When walked he doesn’t pull on his leash. Symba sleeps on a dog bed or with a family member. He eats dry Pedigree dog food 2-3x a day (sometimes wet food) and knows “sit”. Intake Behavior: Symba allowed all handling. He allowed to be scanned (-), collared & taken pictures of. Symba didn’t seem to mind being picked up/carried (he even attempted to jump up onto my lap upon meeting) and was placed into a kennel with ease. During his stay with us Symba has had two seizures in his kennel.

Symba was surrendered to the care center by his previous owner. His owner described him as friendly, affectionate, playful, and mellow, with a medium activity level. No behavior issues were reported aside from occasional accidents in the home, which may be due to age. Please see the Profile memo for a full report. Due to Symba’s current medical condition and repeated seizures in the care center, a full behavior assessment is not appropriate at this time. We recomend placement with a New Hope partner who can address his medical needs and then re-evaluate behavior in a stable home environment.

9/20/16 Hx: History of seizures, possibly on medication for them (client said that the cost of medication was too high for him, but didn’t state what meds the pt was on or what the meds were for). Pt had three seizures yesterday after initial exam. S: Stands and pants, allows all handling, doesn’t seem to pay attention to handler O: BAR-H, BCS 4/9, MMs pink and moist EENT: No discharge OU or AU. Nasal planum has many deep cracks and fissures. Severe tartar, gingivitis and gingival recession. Matted fur on face. PLNs: Not enlarged. H/L: NSR, NMA. Pants or breathes loudly through nose. Abd: Soft, no pain on palpation, no masses palpated M/S/I: No swelling or injuries noted, but pt is unwilling to walk. Generalized muscle wasting over hind legs. Moderate medial buttressing bilateral stifles, no crepitus. Matted fur on legs and on hind end. UG: Male intact, testicles soft and symmetrical. A: 1. Seizure history – R/O brain tumor vs. idiopathic seizure disorder 2. Matted 3. Severe dental disease 4. Muscle wasting on hind legs – R/O arthritis and disuse Short-term prognosis: Poor-guarded P: Monitor for seizures. Consider EHR. 1088
