DVM Intake Exam; Estimated age: reported to be 5 years; Microchip noted on Intake? ; History :owner surrender; Subjective:BAR; Observed Behavior -energetic and friendly; Objective ; BCS 5.5/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam:mm pk, moist; CRT <2 sec; mild to moderate tartar/staining, significant overbite, drools and hangs tongue out of mouth (seems normal for p); PLN: No enlargements noted; H/L: eupnic, clear lung sounds; grade IV-V/VI heart murmur; ABD: Non painful, no masses palpated; U/G: male intact, 2 scrotal testicles; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal:grossly normal; Assessment; Heart murmur; Prognosis:fair; Plan: recommend cardiac work up including chest x-rays, blood pressure, ECG +/- echocardiogram; suspect underlying heart disease given severity of murmur; SURGERY:permanent waiver due to heart murmur
