DVM Intake Exam; Estimated age:8-9y; Microchip noted on Intake?n; Microchip Number (If Applicable): History :stray. very hungry/thirsty; Subjective: emaciated, covered with yellowish crusts esp eyes and belly; Observed Behavior -BAR, curious, allows exam; Evidence of Cruelty seen -poss neglect; Objective ;T = P =60; R =wnl; BCS 1/9; EENT: ou sclerosis, ears clean, yellow ocular crusts; Oral Exam:heavy dental tartar; PLN: sunmandbular ln enlarged; H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: no SS seen; MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty coat- crusty lesions; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: emaciation, rule out underlying disease (eg diabetes) vs lack of nourishment; skin lesions may be due to poor husbandry; Prognosis:fair; Plan:small frequent meals; cbc/chem /t4; postpone intake tech stuff until tomorrow; malaseb bath; SURGERY: Permanent waiver due to age, condition

Summary (1): Sweet face was surrender as a stray so her past behavior with other dogs is unknown.
3/21: When off leash at the Care Centers, Sweet Face briefly acknowledges the greeter before relieving herself. She displays a lean back posture with a tense face, then quickly air snaps, when the greeter solicits play.
