Medical Summary Microchip: negative, placed 981020025095070 Sex: intact female Age: appx 8-10y Mentation: QAR, dehydrated (prolonged turgor, tacky mm) Eyes: heavy d/c OU, possible KCS in appearance, moist dermatitis under OD from large amount of debris and d/c (shaved of) Ears: clean Nose: no d/c Teeth: ddz (heavy staining, wear, some missing, underbite) If abnormal BCS: emaciated 3/9 Skin: some areas of dermatitis, otherwise WNL Hair Coat: mats throughout, “boots” mixed with overgrown entwined nails, shave most of body and requires “touch up” Declawed: N/A, overgrown, embedded nails throughout but most notably in RHL (puncture had large maggots, all cleaned out during shave, feet soaked in dilute novalsan) Any injuries: see above Behavior: allowed all handling, vocal and uncomfortable when restrained but stopped resisting and calmed when toweled Medication: N/A, awaiting DVM rx in a.m., preventatives given

RE EXAM 09/19/17: S/O; QAR, allows handling; moderate appetite, seems to free feed through the day; mm pk, tacky; green mucoid nasal discharge; OS-cornea appears dull, corneal defect in mid field; eupnic, slightly harsh lung sounds; soft abdomen; ambulatory paraparesis with significant offloading onto forelimbs and kyphosis of TL spine; discomfort on palpation of TL junction; wound on dorsal RH has appropriate tissue healing with no active discharge/swelling; A; Geriatric; KCS with corneal ulcer; Dental Disease; Nasal discharge-r/o CIRDC vs dental disease vs chronic rhinitis (inflammatory, infectious, neoplastic) Paraparesis/TL pain-suspect chronic IVDD; P; move to medical isolation; d/c amoxiclav; doxycycline 1 ml PO SID x 14 days; LRS 100 ml SQ SID x 3 days; gabapentin 250 mg/5 ml oral suspension: 0.6 ml PO BID x 5 days
