LN H/L: left parasternal heart murmur grade V/VI, CRT ~ 3 sec, Lungs clear, eupnic ABD: soft, non painful, no masses palpated; palp. abdominal scar U/G: presumed spayed, no mammary development, palp. abdominal scar MSI: emaciated BCS 1/9; ambulatory x 4, skin free of parasites, no masses noted, dry hair coat, thin skin, long tails CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: – poor/emacitated BCS – bilateral cataracts – heart murmur – appearance of multiple underlying chronic conditions/metabolic disorders (r/o heart dz, diabetes, neoplasia, vs other) Plan: – CBC/Chem/UA – rec’ full medical work-up +/-chest rads — rec’ cardiac workup Prognosis: open – depends of severity of conditions and ability to treat, but she has the appearance of longterm chronic illness that will need follow-up medical attention and regular treatment SURGERY: n/a – waived because presumed spayed and health
