Medical Summary Scanned negative BARH – lip curl, snarls, may snap – muzzle used for exam F~ 6 years Ears, nose, throat clear Teeth white, some wear. Eyes – Both eyes red. Skin gouged above eyes by dog’s nails. Right eye appears slightly bulging with fluid behind cornea. White spot showing. Left eye appears corneal scarring/ulceration? Possible glaucoma? Owner states “dog going blind, eyes itching for a year.” VC requested. Coat – thinning all over body. Clean. No flea dirt or parasites noted. Nails extremely long – trimmed. Ambx4 NOSF

Basic Information:
Sunshine is a 6 year old female maltese/chihuahua mix who was surrendered due to lack of time.
Socialization:
Around starngers Sunshine is friednly, outgoing, and plays gently with adults. Sunshine has lived with children ages 3 to 4 years old; around them she is relaxed and plays gently. Sunshine has lived with another small dog; around that dog she was relaxed and played gently. Sunshine has also lived with cats; around them she was relxed and playful. Sunshine has never bitten another person or animal.
Behavior:
Sunshine will use the bathroom indoors if she is not taken out regularly. During thunderstorms and fireworks Sunshine will get nervous. When someone touches Sunshine’s food or bowl while she is eating, takes a treat or toy away, or takes any object away Sunshine is friendly toward them. Sunshine is also friendly when someone gives here a bath, trims her nails, brushes her coat, or restrains/holds her. If an unfamiliar person approaches Sunshine’s family or friends she is friendly towards them.
For a New Family to Know:
Sunshine is described as friendly, affectionate, playful, confident, mellow and independent with a medium activity level. When at home Sunshine likes to stay in the same room as her owners. Sunshine does not usually play with toys. Sunshine will sleep whereever she is most comfortable and was fed dry food from the brand puppy chow on a free feeding schedule. Sunshine will use wee wee pads and prefers to go to the bathroom on the grass. When left alone at home Sunshine is well behaved. Sunshine is not crate trained and doesn’t know any commands. Sunshine slow walks on a leash and dosen’t pull. When off leash Sunshine will wander some but will come when called.
Intake Behavior:
Sunshine was nervous during her intake but warmed up to staff after some time. Sunshine allowed staff to collar,scan for a microchip and take pictures with no signs of aggression.
