Medical Summary DVM Intake Exam Estimated age: 10yrs Microchip noted on Intake? none History : brought in by police Subjective: Observed Behavior – allows all handling, no aggression  Objective P = 120 R = pant BCS 6/9 EENT: OU nuclear sclerosis, mild iris atrophy, ears clean, no nasal discharge noted Oral Exam: severe dental tartar, gingivitis, halitosis, oligodontia PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI, both testicles descended, L testicle much larger than L, non painful. L testicle somewhat firmer than the R MSI: Ambulatory x 4, fleas all over, no masses noted, thinned furcoar all over, especially on tail shaft, paws saliva stained CNS: mentation appropriate – no signs of neurologic abnormalities Assessment fleas- advantage applied yesterday severe dental/periodontal dz, oligodontia fur- r/o underlying cushing’s, other testicles: r/o normal for p, testicular tumor, other saliva stained fur-r/o underlying allergies iris atrophy and nuclear sclerosis Plan rec bathe p if fleas not gone in 2 days to “drown” fleas” rec cbc/chem/t4 and consider ACTH stim, possible AUS to look for Cushing’s rec dental px with extractions, consider neuter with biopsy sent out for histopathology Prognosis: fair SURGERY: Permanent waiver due to age

As a Star Trek’s fan, the name Sulu attracted me right away. Taking out the ten year  old canine bearer of the name was no disappointment…Sulu is really a nice little guy, always at the front of his kennel and ready to go…Although an elderly, Sulu is a very good walker and stairs climber. He does his business outside his den which keeps is quarters clean. Despite the fact that his teeth have seen better days, Sulu has a great appetite and comes on the lap to ask for more treats . He  also quite often  requests hugs by tapping on my knees…. He is very responsive to orders and is really a pleasure to spend time with,  making  him a very nice, calm while lively and loving companion. Sulu comes with expected ailments connected to the  aging process and probably suboptimal care but there is nothing better to change things around than a new  and loving master or family. Come and meet Sulu at the Manhattan Care Center and take him home.
