Medical Summary Estimated age: 8-15 years based on dentition and overall appearance Microchip noted on Intake? Scanned neg on intake History : Stray, found outside, no hx Subjective: Quiet, alert, stands on exam table, allows all handling Objective BAR-H, MMs pink and slightly tacky, BCS 4.5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Severe tartar buildup, missing all maxillary incisors PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male intact, left testicle is present in scrotum and right testicle is not palpable MSI: Ambulatory x 4, many live fleas crawling on pt. Skin of ventrum is hyperpigmented. No masses noted. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment: 1. Severe dental disease 2. Fleas! 3. Cryptorchid 4. Hyperpigmented skin – R/O flea allergy dermatitis vs. other Plan: 1. Capstar today during exam 2. Recommend dental cleaning with rads and extractions as necessary 3. Recommend bathing, monitor skin – if hyperpigmentation doesn’t resolve with regular flea prevention and bathing, then rec evaluation for other causes of dermatitis 4. Recommend neuter outside of ACC; cryptorchid testicles (undescended testicles) are at high risk for becoming cancerous. It is also a genetically heritable condition so Stratford should not be bred. Prognosis: Fair SURGERY: Permanent ACC waiver due to age. 1088

Stratford is the cream version of the Chihuahua I adopted years ago from the care center, also at age 10. Gordy had a very special place in my heart and whoever looks and acts like him attracts me like a magnet..Stratford is no disappointment…He shakes as a Chihuahua usually does but the arms and the lap calm him down pretty quickly…That is where he belongs, that is where all the Chihuahuas belong as needy and loyal best friends. Stratford walk well on the leash, does some business on the way and accepts the petting of staff passing by. He does not react to other dogs and even wags his little tail as he meets a big gal…Stratford is very dear to me and I so would wish for you to meet him and fall in love with him. Stratford is waiting for you at the Manhattan Care Center
