Medical Summary DVM Intake Exam Estimated age: ~ 3 yrs Microchip noted on Intake? scan negative History : owner surrender Subjective: BARH **aggressive, unpredictable behavior – unable to handle or give full PE – provided limited intake PE under partial sedation** Observed Behavior – calm allowed hanlding on intake but becomes reactive, does not show much outward signs of aggression but will bark for attention and also lunges to bite when approaching, walking by and when members of staff turned their backs to the dog. Objective BCS 6/9 EENT: Eyes clear, no c/s, no nasal discharge noted Oral Exam: not performed PLN: not examined H/L: not examined ABD: not examined U/G: female, enlarged vulva – presumed intact MSI: Ambulatory x 4, no parasites observed, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: apparently healthy – but will need full PE to assess if possible/behavior allows Plan: no treatment at this time Prognosis: good SURGERY: Okay for surgery
