Intake condition UNSPECIFIE Intake Date 04/11/2017, From NY 10029, DueOut Date 04/14/2017,

Medical Behavior Evaluation GREEN
Medical Summary DVM Intake Exam Estimated age: 12 yrs Microchip noted on Intake? neg History : brought in as a stray Subjective: QAR Observed Behavior – nice, old boy; easy to examine; prefers lying down; gets winded easily  Objective T = not taken P = WNL R = panting BCS 6/9 EENT: OD-severe enopthalmia/microopthalmia with poss bulbar conjunctival mass near medial canthus; suspect globe is present in cavity but unable to visualize Oral Exam: mm pk, sl tacky; CRT <2 sec; significant wearing, discoloration, tartar on teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2; panting with normal effort but does get winded easily when walking around-suspect may be due to pain from OA and not primary lung dz; normal lung sounds in all fields ABD: tense on palpation of cranial abdomen; no obvious masses or other abnormalities appreciated but palpation difficult U/G: intact male; 2 normal descended testicles present MSI: bilateral grade I FL lameness with carpal crepitus; bilateral grade I HL lameness with offloading onto RH; muscle atrophy of thigh muscles bilaterally; thickening of R stifle with no positive drawer; mild thickening of L stifle with mild pos drawer dandruff, dull hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: thickening of rectum; rectal not performed Assessment 1. OD microopthalmia/abnormalities-r/o neoplasia vs cherry eye with concurrent microophthalmia 2. Osteoarthritis-carpal and stifle, suspect old R CCL injury and more recent L CCL injury Plan Rec’d exploratory of orbital cavity with poss enucleation/biopsy of tissue at time of neuter Monitor appetite, attitude, water intake over the next 24-48 hours; if any concerns then rec’d CBC/chem Rimadyl 75 mg SID for generalized OA and potentially additional pain medications suchs as gabapentin, tramadol, or amantadine long term Prognosis: guarded due to age, ocular abnormalities and generalized OA SURGERY: Okay for surgery as long as no signs of systemic illness noted during the next 24-48 hours
