Medical Summary DVM Intake Exam Estimated age: reported to be 4 by previous owner Microchip noted on Intake? History : owner surrender; owner reported that she bit owner’s mother but would not give more specific reasons Subjective: BAR Observed Behavior – a little nervous but allows handling; warms up quickly and gave kisses Objective BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk, moist; CRT <2 sec; muzzled as a precaution for exam PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G:female spayed; tattoo noted on abdomen MSI: Ambulatory x 4, skin free of parasites, no masses noted; mild erythema along dorsum with mild pruritus; few papules inguinally and on abdomen CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Dermatitis Plan Rec’d medicated bathing twice weekly after placement Prognosis: good; dermatitis is likely related to underlying allergies which are very common in this breed and may require intermittent symptomatic treatment; if signs are noted to be more frequent or severe, then diet trial +/- allergy testing may be indicated SURGERY: spayed
