09/09/2017 PET PROFILE MEMO
09/09/17 18:03 Basic Information Star is a seven and half year old, brown and white colored, large female dog. She was surrendered by owner for personal reasons. Owner found her as a stray and had her for seven years; and, she has no signs of illness or injury. Socialization Star is very friendly and outgoing around strangers. She lived at home and spent time with children ages five and seven and was relaxed and respectful around them. She is gentle when playing with children and adults. She did not live or spend time with other dogs or cats and, has never bitten a human or another animal. Behavior Star is generally well behaved. She gets terrified of loud noises-she shakes and whimpers. She growls if you touch her food or bowl while eating; is friendly if you take her treat or toy away and is not bothered if pushed off furniture or disturbs while sleeping. She is friendly when given baths (she loves taking baths), and isn’t bothered if you brush her coat. She is friendly if unfamiliar faces approach owner or home. For a new Family to Know Star is a friendly, affectionate, playful and confident dog with a high energy level. She is very loyal protective of owner and his family. She’ll be in her favorite spot at home and likes to play balls, fetch and tug. She was kept mostly indoors and slept on her dog bed on the couch; fed both dry and wet dog food (Pedigree) twice daily and has no favorite treats. She is house trained and goes potty on grass when outdoors. She is well behaved when left alone at home; was never crated and understand the commands sit, come, stay, run and catch. She goes running every day; pulls lightly on the leash and stays close to your side when off leash. Behavior during Intake Star had a very relaxed body with an open mouth and a wagging tail. She took treats, allowed petting and kissed counselors face. Star allowed all handling.

09/12/2017 BEHAVIOR EVALUATION – NH ONLY
Exam Type BEHAVIOR
KNOWN HISTORY: Full Profile Date of intake 9/9/2017 Unaltered female, owner surrender (in previous home for seven years) Previously lived with: Adults, has spent time with children ages 5 and 7 Behavior toward strangers: Friendly, social and outgoing Behavior toward children: Relaxed and respectful, plays gentle Behavior toward dogs: Unknown Behavior toward cats: Unknown Resource guarding: Will growl if you touch her food bowl while eating, but friendly w/toy or treat Bite history: None reported Housetrained: Yes Energy level/descriptors: Friendly, affectionate, playful & confident dog with high energy Other notes: Star is terrified of loud noises, will shake and whimper. She loves taking baths. Star went running daily with owner, knows the cues ‘sit, come, stay, run, and catch’. SAFER ASSESSMENT: Date of assessment 9/12/2017 Summary: Star is difficult to remove from her kennel. She has allowed almost no handling from her caretakers here at the center. Out of concern for her stress levels, we feel Star is not a great candidate for a handling assessment at this time. Please see her owner surrender profiel for a full summary of her behavior in her previous home. INTAKE BEHAVIOR: Date of intake 9/9/2017 Star was very relaxed upon intake, open mouth, wagging tail, allowed all handling. MEDICAL BEHAVIOR: Date of initial 9/10/2017 During her initial medical exam, Star was extremely stressed due to environment so exam was limited. ENERGY LEVEL: Her owner reports Star as having a high level of activity. In the care center, Star is extremely fearful and not allowing any handling- activity is very low.

RECOMMENDATIONS: New Hope Only _X_Place with a New Hope partner: Due to her extreme fear of loud noises, Star has not acclimated well to the kennel environment and has allowed very minimal handling since intake. We recommend placement with a New Hope partner who can provide any necessary behavior modification (force-free, positive reinforcement based) and re-evaluate behavior in a stable home environment before placement into a permanent home. Potential challenges: _X_Fearful: Star has displayed fearful behavior in shelter, she displays no social behavior towards handlers and attempts to increase distance and avoid contact. Because she is so uncomfortable, Star does not tolerate any form of human-touch and has attempted to snap upon direct contact. _X_Kennel presence: While we cannot be certain if this behavior will appear in any other contexts, in highly emotionally charged or stressful/loud environments Star may show behavior similar to what she is currently demonstrating in her kennel.
