Subjective: Quiet, slightly depressed, geriatric dog Observed Behavior – quiet, friendly Objective T = (not done) P = 168 R = 36 BCS 2/9 EENT: OU – mild mucoid ocular discharge; AU ears clean; no nasal discharge noted Oral Exam: missing all teeth PLN: No enlargements noted H/L: nsf = NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: nsf = Non painful, no masses palpated U/G: neutered male ( no palp. testicles) MSI: general muscle waiting, underweight/poor BCS, amb x4 with sitff gait, no pain/response to flex/extend of limbs; mod matted coat – worse around face/eyes – needs bath/grooming; skin free of parasites, no masses noted CNS: quiet, slightly dull mentation – no signs of neurologic abnormalities observed Rectal: not performed Assessment – Geriatric — missing all teeth — poor BCS Plan – rec’ gen BW + UA for baseline and since owner reports “illness” – rec’ NH placement Prognosis: fair/guarded if underlying illness SURGERY: neutered
