DVM Intake Exam: Estimated age: 7yo  Microchip noted on Intake? neg History : stray Subjective: BAR, very friendly and good for medical handling.Observed Behavior:   Objective  T =. P = wnl. R = eup. BCS 5/9EENT: Eyes clear, ear canals dry and bit flaky with mild debris AU, no nasal or ocular discharge noted Oral Exam: mandibular prognathism, moderate dental disease PLN: submandibular LNs mildly enlarged  H/L: Grade V/VI left systolic HM. NSR, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated U/G: intact male, 2 testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, fair hair coat, hypotrichosis and epidermal collerettes around neck and along dorsum.  CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: normal externally Assessment: Grade V/VI HM
geriatric,dental disease, superficial pyoderma,otitis externa. Prognosis: guarded pending echocardiogram  Plan: ear cleaning. Otibiotic AU. convenia 8mg/kg SQ once
recommend echocardiogram asap to evaluate heart murmur  SURGERY:  Permanent waiver due to heart murmur
