Medical Summary 8/23/16 Microchip scan = pos. # 911002001298923 O: QARH. Very sweet and gentle, allows all handling. MM=pink, moist, CRT<2 s ORAL: Severe calculus, gingivitis, oral odor. EENT: OU – Marked mucopurulent ocular discharge, crusting in fur below eyes. Cleaned away discharge – sclereal injection and mild corneal opacification OU. H/L: No murmurs. ABD/UG: Female. Palpation WNL. INTEG: 3 cm soft sq mass L ventral cervical region. Alopecia ventral neck. MS: Very thin body condition. A: Suspect KCS OU. Severe dental disease. Thin condition – R/O secondary to dental disease, metabolic disorder, malnutrition. Geriatric dog. Prognosis fair. P: STT = 0 mm/min OU – confirms KCS. Cleaned OU and applied Neopolybac ophth ointment, artificial tears. Rx TAB with HC ointment. Apply OU BID x 2 weeks. Continue as directed. Rx Puralube ointment. Apply OU BID x 2 weeks. Rx Optimmune ointment. Apply OU BID x 2 weeks. Treatment will need to continue life long. Recheck weight, eyes 7 days.
