DVM Intake Exam: Estimated age: 10-12 years based on PE. Microchip noted on Intake? scanned positive by LVT on intake. MC# History : Stray Subjective / Observed Behavior – BAR, growled when lifted. Opened mouth and lifted lip for oral exam. Attempted to flee and growled during blood draw. Objective: BCS 5/9. EENT: Nuclear sclerosis, OU; mild exudate, AU; no nasal or ocular discharge noted Oral Exam: dc 3/5; pd 3/5 PLN: No enlargements noted H/L: 4/6 L systolic murmur ausculted; CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: neutered male. MSI: Ambulatory x 4, skin free of parasites, dirty haircoat. 1inch soft, black mass along the dorsal L shoulder. 3inch SQ soft, freely moveable mass along the ventral thorax. 1inch SQ, attached mass along the right caudolateral thorax. CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: grossly normal. Assessment: multiple masses
dental disease geriatric nuclear sclerosis heart murmur Prognosis: fair to good. Plan: ear cleaning and claro otic application — performed at intake. rec cardiology consult once placed / adopted . rec needle biopsies of masses once placed / adopted. SURGERY: already neutered. S/O – Green mucoid nasal discharge. Pt QAR. No ocular mucoid discharge. No coughing. A – CIRDC. P – Move to iso- Doxycycline 10mg/kg SID for 14 days- Cerenia 2mg/kg SID for 3 days
