Medical Summary DVM Intake Exam Estimated age: 6-8–hard to assess due to appearance at admission Microchip noted on Intake? n History : stray Subjective: severe matting–feet not visible. severe chronic changes to eyes due to large cherry eyes. severe chronic ear disease Observed Behavior – wags tail, allows all treatments, seeks petting Evidence of Cruelty seen – yes, neglect Objective P = 80 R = wnl BCS 4/9 EENT: eyes: hyperpigmentation and scarring. OU large cherry eyes, covered in purulent d/c. KCS may be present too. Ears are end stage: stenosis and irritation/proliferative tissue which obstructs canals Oral Exam: moderate/heavy tartar H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: felt spay scar MSI: stumbles due to matts on feet. matting so severe skin cannot be assessed CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: severe chronic ear and eye disease, matted Plan: shavedown is a priority. TAB OU long term until cherry eye surgery. Otibiotic bid x 28 days. may need bilateral teca sx. recc STT after a few weeks of treatment Prognosis: good . issues with ears and ears are chronic though
