Medical Summary DVM Intake Exam Estimated age: 4-6 years Microchip noted on Intake? neg History : stray Subjective: QAR Observed Behavior – a little anxious but docile, allows handling; strainguria noted on intake; urine appeared slightly blood tinged Objective BCS 4/9 EENT: Eyes clear with mild crusting at medial canthus, ears clean, no nasal discharge noted Oral Exam: mm pk, sl tacky; CRT <2 sec; moderate tartar, mild staining PLN: No enlargements noted H/L: no murmurs, SSFP; intermittently increased resp rate with normal effort; normal lung sounds in all fields ABD: Non painful, no masses palpated U/G: female intact MSI: ambulatory x 4 with slightly stiff/stilted gait in forelimbs no significant spinal pain hair coat slightly overgrown dark brown/black crusting material along bottom of all paws and on forelimbs CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Hematuria-r/o UTI vs urinary stones Plan amoxiclav 1.5 ml PO BID x 14 days rimadyl 12.5 mg PO SID x 3 days rec’d rads to r/o urinary stones Prognosis: good to excellent SURGERY: Okay for surgery
