Medical Summary Microchip: 985112003890700 Sex: spayed female, thought to have seen scar Age: appx 7y Mentation: BARH, painful Eyes: clear Ears: clean Nose: no d/c Teeth: moderate staining, would not allow check of back teeth If abnormal BCS: closer to BCS 3.5 Skin: WNL Hair Coat: WNL Declawed: N/A Any injuries: dart wound to left side caudal to hip point but low on abdomen (at triangle of hip and knee) at around 3pm 01/28; sedated with 0.9ml dexdormitor IM (reversed with same amount of atipamezole) and removed with blade after topical lidocaine 2%, packed with TAB ointment and per 0915 gave 3.25ml PPG SQ Behavior: warmed slowly, listened to “sit” command, was painful so resisted some of exam; history of darting w/o behavioral indication (warning from owner) Medication: ABx (PPG 3.25ml) and topicals; preventatives given
