Medical Summary DVM Intake Exam Estimated age: geriatric – 7-9 years Microchip noted on Intake? no History : found stray, no medical history Subjective: QAR, euhydrated, very nervous Observed Behavior – struggled for exam and tried to bite a little for oral exam – muzzled Objective BCS 4/9 EENT: Moderate nuclear sclerosis OU, ears clean, no nasal discharge noted Oral Exam: severe dental tartar and gingivitis PLN: No enlargements noted H/L: NSR, NMA, strong synchronous pulses, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact female MSI: Ambulatory x 4, skin free of parasites, 0.5 cm mammary masses right cranial and left middle quadrants, 1 cm laceration below eye with purulent discharge CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: geriatric intact female, multiple mammary masses, infected laceration below eye Plan: cleaned laceration and applied neo-poly-bac. Start Clavamox drop 0.53 mL PO BID x 10 days and apply neo-poly-bac to laceration BID. Seek New Hope placement Prognosis: fair SURGERY: Permanent waiver due to advanced age
