DVM Intake Exam; Estimated age:9-10y; Microchip noted on Intake?n; History 😮 surrender; Subjective: large mass movable in sq 2″ diameter behind right ear. numerous wrt like masses on thorax.; Observed Behavior -wags tail, nervous shaking; Objective; T =; P =80; R =wnl; BCS 5/9; EENT: od sclerosis OS opaque cornea most likely non fucntional eye, ears clean, no nasal or ocular discharge noted; Oral Exam:heavt tartar and dental dz; PLN: No enlargements noted; H/L: 4/6 heart murmur; ABD: Non painful, no masses palpated; U/G:neutered; MSI: Ambulatory x 4, skin free of parasites,; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: heart murmur, large mass on head, numerous wart, eye disease; Prognosis:fair; Plan:heart needs to be evaluated before mass removal/enucleation considered. mass may be benign
